Join the Practice

Registration

If you live within our practice area you can register by completing the relevant registration form plus the data sharing and accessibility forms.

The necessary forms are available from reception or can be downloaded as PDF file – see below.
Please note that if you are returning your forms in person, where possible please do so between 10am and 4pm Tuesday to Friday.

You can also send your online request to join the practice through the online form below. Once  our request is processed,  Our admin team will contact you soon.

Proof of identity

  • Patients over the age of 16 years can have online access to their medical record and additional facilities such as ordering repeat prescriptions and the booking of some appointments. We require proof of identity to be able to give you online access to your medical record. At the time of registering or applying for online access, please supply proof of identity and of current address. PLEASE NOTE: these should be separate documents, for example we cannot accept a driving licence as both photographic ID and proof of address.  Acceptable documents can be:
    👉 Photo ID: passport or photo driving licence
    👉 Proof of address: bank or other financial statements, but not bills

New patient check

Newly registered patients are invited to arrange a consultation with a healthcare professional – this is an opportunity for us to obtain important background medical history prior to your medical notes arriving.  When you come for your medical please bring along a small specimen of urine. We do need to have your details logged onto our computer system before you are seen for an appointment.

Your named GP

The surgery provides all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them. We need to divide our patients evenly between all of our doctors, so they can manage their workload. Therefore to do this fairly and impartially, we have selected patients by surname to match to a doctor.

We know that some patients will wish their named GP to be a particular doctor. Where a patient expresses a preference as to which doctor they have been assigned, the practice will make reasonable efforts to accommodate this request, but we would like to reassure patients that this system does not prevent you from seeing any GP in the practice. Your named GP will not be available at all times. You do not need to take any further action and will be informed of your named GP in due course. In the meantime, if you wish to be told the name of your accountable GP, please ask the receptionist when you are next in the surgery.
